LAXOR’s full Notice of Privacy Practices, is available in PDF format. It can be downloaded by clicking here, and it describes how information in your LAXOR® Personal Health Record may be used and disclosed.

All information stored within the LAXOR, Inc. (LAXOR®, LAXOR) Personal Health Record (PHR) is confidential. It will not be shared, distributed or marketed to any individual, provider or other entity without your consent except as required by law.

A health professional with whom you do not have an established LAXOR relationship can access your LAXOR Personal Health Record for a limited period of time if you have enabled such access ahead of when such emergency access is needed.

LAXOR reserves the right to change our privacy practices and to make the new provisions effective for all health information in your personal health record. Our current Notice of Privacy Practices in PDF format can be downloaded by clicking here. You will also be notified at the email address we have on file for you when we make changes to it.

Members who have questions or who would like additional information use our contact form or call 1-866-233-3606.

A member who believes that his or her privacy rights have been violated can file a complaint with our Chief Privacy Officer by using our contact form or by calling 1-866-233-3606. There will not be retaliation of any kind for filing such a complaint.

All LAXOR personnel who have a need to access the network and the PHR application do so only when necessary and only in so far as their work requires such access. All LAXOR personnel have signed confidentiality agreements that meet industry standards.

Security


The LAXOR Personal Health Record uses standard 128-bit SSL (Secure Socket Layer) encryption. Your health information is stored on LAXOR-owned and managed state-of-the-art servers (computers) located in a Tier-1 highly secure hosting facility. In addition, your health information is protected through the use of managed firewall's and all connections to the LAXOR Fax Intake Service are done using a virtual private network (VPN).

Your health information is backed up nightly, and a full working version of the PHR is maintained at a separate off-site secured hosting facility.
